Policies & Forms

The following are policies and procedures and other helpful information.

Questions

If you are interested in scheduling an appointment for yourself or for a dependent, or to ask general questions about therapeutic services or classes, you may call 202-230-1075 or contact me.

Email Consultations

Please do not request therapeutic advice via e-mail; because of the lack of confidentiality and because the context of an ongoing therapeutic relationship is essential to providing effective guidance, I cannot respond to such questions.

Phone & Online Consultations

If you live anywhere included in the Psypact telehealth agreement, sessions can be conducted by telephone or online via Zoom or Doxy. In all other locations, feel free to call to discuss other ways we might work together.

During the pandemic, I am not seeing anyone in person.

Confidentiality

Our communications are kept in the strictest confidence except for the extremely rare event in which I would be required by law to report concerns about danger to self or danger to another.

New Clients

Prior to your first appointment, please download and complete the Intake Form and email it to me at jannicholsonedd@gmail.com.

Payment/Insurance

Payment for sessions is required at the time that services are provided.  Payments are via Venmo, Zelle, or Ivy Pay (which is encrypted and allows you to use a credit card or HSA). A check can be mailed as well. If you request it, I provide a fee slip after each session for you to submit to your insurance company. While I do not participate with any insurance companies, if you are allowed to see an out-of-network provider, you may receive reimbursement. It is your responsibility to check with your insurance company about your benefits.
